Indications/Uses
Neuroradiology (myeloradiculography, cisternography & ventriculography), angiography [cerebral, coronary, selective visceral & peripheral arteriography; thoracic & abdominal aortography; angiocardiography; venography; digital subtraction angiography (DSA), DSA of cerebral, peripheral or abdominal arteries], urography (IV urography). Contrast enhancement in computed tomography (CT) scanning, arthrography & fistulography.
Dosage/Direction for Use
Myeloradiculography, cisternography & ventriculography 5-15 mL of 300 mg iodine/mL. Cerebral arteriography 5-10 mL (bolus) of 300 mg iodine/mL. Coronary arteriography 8-15 mL (bolus) of 370 mg iodine/mL. Angiocardiography, thoracic & abdominal aortography 1-1.2 mL/kg of 370 mg iodine/mL. Selective visceral arteriography Depending on exam (300-370 mg iodine/mL). Peripheral arteriography 40-50 mL of 300-370 mg iodine/mL. DSA Depending on exam (150-370 mg iodine/mL). Venography 30-50 mL of 300 mg iodine/mL. Urography 30-50 mL of 300-370 mg iodine/mL. Contrast enhancement in CT scanning 0.5-2 mL/kg of 300-370 mg iodine/mL. Arthrography, fistulography Depending on exam (300 mg iodine/mL).
Contraindications
Waldenström's macroglobulinemia. Multiple myeloma. Severe liver & kidney diseases.
Special Precautions
May cause untoward side effects, anaphylaxis; skin & severe CV reactions. Never mix other drugs w/ contrast medium soln. Hyperthyroidism. Risk of transient hyperthyroidism or hypothyroidism. Patients scheduled for thyroid exam w/ radioactive iodine tracer. Keep soln away from strong light. Do not use in the event of crystallization. Use immediately once opened. Pregnancy. Do not limit fluid intake before administering a hypertonic contrast soln when examining small childn or babies; correct existing water & electrolyte imbalance. Possible decreased thyroxine, triiodothyronine & increased TSH level in infants especially preterm. Neuroradiology: May be contraindicated in patients w/ history of epilepsy; presence of blood in CSF. Discontinue seizure threshold-lowering drugs (eg, neuroleptics; analgesics, antiemetics, antihistamines & sedatives of the phenothiazine group) at least 48 hr before administration & not to be resumed <12 hr after procedure completion. Angiography: Advanced arteriosclerosis, HTN, heart failure, major systemic diseases & recent cerebral embolism or thrombosis. Right heart insufficiency & pulmonary HTN may precipitate bradycardia & systemic hypotension. Paed roentgenology; inj into right heart chambers of cyanotic neonates w/ pulmonary HTN & impaired cardiac function. Carefully position the tip of catheter during aortic arch exam & abdominal aortography. May cause painful reaction in the involved limb in peripheral arteriography. Minimize risk of procedure-related thrombosis & embolism by paying meticulous attention to angiographic technique & catheter flushing when performing vascular catheterisation procedures.
Adverse Reactions
Risk of anaphylaxis, skin & severe CV reactions. Endocrine disorders. Neuroradiology: Headache, nausea, vomiting & inj site pain; fever; muscular spasms or generalized convulsions. Angiography: Heat sensation in face & neck, headache, bradycardia w/ systemic hypotension, severe neurological sequelae. Arthrography/fistulography: Irritative manifestations superimposed on existing tissue inflammation.
Drug Interactions
Neuroradiology: Avoid seizure threshold-lowering drugs (eg, neuroleptics; analgesics, antiemetics, antihistamines & sedatives of the phenothiazine group).
